Today's presentation by Carrie Reardon, AVP Patient Care at Adirondack Health http://adirondackhealth.org discussed an initiative which provides health care that is a systematic approach that recognizes increased risk factors faced by aging patients while preventing needless decline while in hosptial and restores health. It is a system that elicits patient goals and preferences while identifying, coordinating and supporting family caregivers. Clinical staff are specifically trained and expert in the care of older adults.
